GDM

Gestational Diabetes Mellitus (GDM) or simply gestational diabetes is a condition in which pregnant women develop high blood sugar. In gestational diabetes, women have normal blood sugar levels before pregnancy but the blood glucose levels rise during pregnancy (usually between 24th and 28th week). The blood sugar levels usually go down to normal after the birth of the baby.

Symptoms:

The symptoms of gestational diabetes are often very mild and hence hard to notice. Some of the most common symptoms of GDM are:

  • Excessive fatigue
  • Blurred vision
  • Excessive urination
  • Excessive thirst
  • Increased appetite

Causes and risk factors:

Though the exact cause of gestational diabetes is not known, most experts believe it is linked to hormonal fluctuations during pregnancy. During pregnancy, the body produces a larger number of hormones like human placental lactogen (hPL) and other hormones which create insulin resistance. Though these hormones are very important for pregnancy, these make the body insulin resistant leading to an increase in the blood sugar levels.

Women with the following risk factors are more likely to develop GDM:

  • High blood pressure
  • A family history of diabetes
  • Women who are overweight during or just before pregnancy
  • Past history of gestational diabetes
  • Have conditions associated with insulin resistance like PCOS etc.

Diagnosis:

To diagnose the condition, doctors screen the patient and monitor the blood sugar levels, especially in between weeks 24 and 28. For diagnosis of GBD, the doctor may perform a one-step or a two-step test which will require you to consume a certain concentration of glucose solution or carbohydrate and your blood sugar levels will be measured an hour or two later. If the test is a two-step test, a follow-up blood test is done on a different day.

Treatment:

The treatment depends on the blood sugar levels found during the tests. If you are diagnosed with GDM, the doctor will ask you to monitor your blood sugar levels regularly, especially before and after meals. The doctor may also ask you to follow a specific diet and follow an exercise regime. Diet and exercise have a great impact on controlling blood sugar levels.

In some cases, the doctor may also ask you to take medications and insulin injections until birth to keep the blood sugar levels in control.

Complications:

Women who have gestational diabetes are at a higher risk of developing type-2 diabetes later in life. If gestational diabetes is poorly managed, it also poses some health risks for the baby like:

  • Increased birth weight
  • Low blood sugar levels
  • Breathing problems
  • Higher risk of developing diabetes later in life

Though gestational diabetes cannot be prevented in all the cases, the risk of developing it can be significantly reduced. Maintaining normal body weight, eating a well-balanced and healthy diet and staying physically active can help in preventing gestational diabetes to a significant extent. Staying physically active and performing very light exercises like walking during pregnancy can also reduce the risk of developing GDM.
